CBCT & digital radiograph services
We are pleased to offer CBCT and digital radiograph services. We accept imaging-only referrals and will provide the necessary scans for your external review and diagnosis.
If you need a specific field of view, slice protocol, or rendering style for your case, please indicate that on the referral form so we can capture and export the scan to your specifications.
Scans are exported in DICOM or other standard formats and shared securely with your office.
Referral process & reporting back to your practice
We ensure seamless continuity of care through detailed reporting and patients back to your practice.
After the consultation visit, your office receives a written summary covering the working diagnosis, the recommended treatment plan, and — for cases where a procedure is performed at our office — a procedure note and any imaging exported in a standard format.
Patients are returned to your practice for routine care unless the case requires ongoing co-management at our office.
How to refer a patient
There are three ways to refer a patient to Dr. Yolanda Cruz:
- Online doctor-referral form — the fastest route. Complete the secure form and our office will follow up to schedule the patient and request any additional documents needed.
- Direct Provider Line — call 416-595-5490 and ask for the referring-provider coordinator if you'd like to discuss a case before sending it through.
- Imaging-only referral — for CBCT or digital radiograph referrals where Dr. Cruz is not seeing the patient clinically, indicate "imaging only" on the referral form along with the field of view and indication requested.
What to send with a referral
To make the first visit as efficient as possible for the patient, please send the following with your referral whenever it is available:
- A short clinical summary or referral note identifying the working concern (TMJ, sleep, mucocele, imaging, etc.)
- Recent intraoral and panoramic radiographs, if you have them
- Any prior CBCT scans or sleep studies relevant to the consultation
- A list of current medications and relevant medical history
- For mucocele or soft-tissue referrals, intraoral photographs of the lesion if possible
- The patient's contact information and preferred appointment window

Where patients will be seen
We are located at The Path (Bay St. & Queen St.), 390 Bay St. Concourse Level, Unit C006, Toronto, ON M5H 2Y2.
The office is on the underground PATH walkway at the corner of Bay and Queen, directly accessible from the financial district, the TTC Queen station, and surrounding office towers without going outside — useful for referring patients who work or commute downtown.

A note on scope
Dr. Yolanda Cruz is a general dentist. All clinical work described on this page is performed within the scope of general dentistry. Cases that require oral and maxillofacial surgery, oral medicine, orofacial pain residency-level work-up, or formal sleep medicine diagnosis are referred to the appropriate specialist or physician colleague, with documentation copied to your office.
If you are uncertain whether a given case is within scope for our practice, the simplest path is to send the referral — we will let you know promptly if it needs to go elsewhere.

Dental and medical colleagues — general dentists, dental specialists, family physicians, ENTs, sleep medicine physicians, and allied-health providers (physiotherapists, osteopaths, chiropractors) — can refer patients to Dr. Cruz for TMJ/TMD evaluation, oral appliance therapy for snoring and sleep apnea, mucocele biopsy/laser removal, laser dentistry, and CBCT/digital radiograph imaging.
Yes. We accept imaging-only referrals and will provide the necessary scans for your external review and diagnosis. Indicate "imaging only" on the referral form, along with the field of view or slice protocol you'd like captured.
After the consultation visit you will receive a written summary covering the working diagnosis and recommended plan. For cases that include an in-office procedure, you'll also receive a procedure note and any imaging exported in a standard format. We ensure seamless continuity of care — patients are returned to your practice for routine care unless ongoing co-management is needed.
Reasonable referral triggers include chronic jaw or facial pain, clicking with locking or deviation, recurrent tension-pattern headaches with suspected bruxism, noticeable tooth wear, suspected sleep-related bruxism in a patient who already has an OSA diagnosis, or any case where you'd like a conservative-first opinion before recommending surgery. For oral appliance therapy in OSA, a formal sleep medicine diagnosis (home or in-lab study) is required before fabrication. Please send the most recent sleep-study report and any CPAP-tolerance notes along with the referral. For snoring without a sleep diagnosis, the STOP-BANG questionnaire is a quick triage tool.
